Mythical unique items are items that have a different base from all other items, which means they cannot be gambled.
Mythical unique items can only drop in endgame map areas because they have a drop level of 100+.
Most mythical uniques have special effects or procs that enable unique playstyles or enhance existing ones.
Some mythical uniques have specific drop sources (e.g. the lich king ones). All mythical base items have slightly improved stats. Physical attack weapons generally have 10% more base damage. Spellcaster weapons generally have lower requirements or other small bonuses.
All mythical base items have slightly increased level requirements (usually it's +3 on top of the base item they're designed after; e.g. Titan's Maul for Mythical Titan's Maul).
Mythical uniques cannot be rerolled. What drops is what you get.
